8. Appointment Booking & Communications

• Appointment requests are not confirmed until you receive confirmation from our office
• Communications submitted through the Site are not guaranteed to be reviewed promptly
• Email and standard web forms are not secure methods for communicating Protected Health Information (PHI); use our secure patient portal for any PHI-related communications
• We reserve the right to decline new patient requests at our discretion, consistent with applicable laws

"As Is" Basis
